It's rumoured that he ate parts of Dead's brain. and took fragments of his skull to be made into neclaces. The band have confirmed the latter rumour.

Euronymous was looking for something brutal he could use on the cover, and he actually joined Amnesty Norway to get access to to pictures like this. He eventually found this picture and managed to take a copy of it. The album cover on the original LP is also way more pink than intended, but Euronymous couldn't afford to do it over again. 17
